USW 6787 Wellness Program
HealthPoint Wellness has partnered with USW Local 6787 to bring it's members a quick and easy way to receive a wellness exam. This is part of a broader initiative to create a program for all local unions in our mission to better the health and wellbeing of Indiana's workforce.
USW Local 6787 Members
Please follow the steps below to schedule and receive your wellness exam.
*REMINDER: Spouse's must also receive a wellness exam to qualify for benefits.
Step 1
Request an appointment time through our dedicated USW 6787 link below. If you wish, you can also call us at 219-500-1500.
Step 2
Fill out the online USW HAI Form and HealthPoint patient registration. These are necessary to receive a wellness exam.
Step 3
Your appointment will be confirmed by our staff and final instructions are given. After your visit, HealthPoint will submit your paperwork on your behalf to ensure verification of a wellness visit.
